What is a needs assessment?
A needs assessment is carried out to determine what care services the person you care for may be entitled to.
How do I request a needs assessment for the person I care for?
To request a needs assessment, you will need to contact the Adult Social Care team, or your local Wellbeing Partnership.

What happens during a needs assessment?
During a needs assessment, a trained professional will determine the level of need and how much care a person requires. You may also be asked about your caring role and your ableness and willingness to provide care.
What happens after a needs assessment?
After your needs assessment, the assessor will determine if the person you care for is in need of social care services or if you need carer support. From there, they will refer the person you care for to the appropriate service or signpost you to other services that may be able to help.
